Custom Calendar Forms

The default form in Calendars asks the person booking for their first name, last name, phone number, email address and any other additional information they want to provide. However, there will be times that you need more, less or different information. You can use a custom form for this.

  1. Create your custom form under Sites > Forms > Builder. You can learn more about building forms in the How to Build, Edit, and Utilize Forms article.
  2. Use custom fields and regular fields to collect the information you need. Be sure to add a name and email field so that information is collected. You can learn more about custom fields in the How to Create, Edit, and Delete Custom Fields article.
  3. Once your form is saved, go to Calendars > Calendar Settings and click the three dots next to the calendar information, then click edit.

  4. From the Forms and Payments tab, use the first dropdown under the heading "Select form" to select the form you created in the forms builder.

  5. Click save at the top right hand corner to save your changes.

Now when someone goes to book an appointment on that calendar, they will see the custom form you chose rather than the default form.
