Creating Surveys
Surveys function much like forms do in FG Funnels where you can collect information from your leads and customers with custom fields and such. The main difference between forms and surveys is that surveys have slides, meaning that questions can be spread out across multiple pages versus all in one long page view.

Creating Surveys in FG Funnels
The survey builder is similar to the form builder in look and functionality. You can use custom fields in the survey builder but you can use slides to break up the number of questions shown at one time.
- Access the survey builder through Sites > Surveys > Builder.
Click on "Add Survey" to begin creating a new survey.
Name your survey and start adding elements using the plus sign in the top left hand corner under Add Elements. You can use the quick add elements or use custom fields you have already created.
- Make sure to include an email field in your survey to tie answers to contact records, whether new or existing.
- When you need a new slide, click the plus sign under the slide to add a new slide.
You can rename your slides by hovering over the three dots to the right of the Slide name and clicking settings when the menu appears. Then use the right hand options panel to name your slide.
- Use as many slides as you need by adding questions and custom fields.
- Conditional logic allows for directing respondents based on their answers. The Surveys with Conditional Logic article walks you through more details on using this in your surveys.
- If you are using a field with a type of Checkbox, Single Options or Radio Button, you can choose to disqualify a lead immediately or on submit based on their answers to send them to a different location based on that response.
Use the Styles and Options panel and the Styles Tab to customize your survey's appearance.
Use Styles and Options -> Options to set up either a message that is shown or a URL that is directed to upon submission. You can also set up a message for a URL for someone who is disqualified.
Adding Surveys to Pages and Viewing Submissions
There are a few different ways that you can share a survey to be filled out by leads and customers.
Within an FG Funnels Page
You can add the Survey Element to any funnel or website page within FG Funnels by clicking and dragging the Survey element from the Add New Element panel. Select your Survey from the dropdown.
Leave the redirect action as "Use Action from the Form Builder" if you have set up On Submit and On Disqualification messages or URLs in the survey builder.
Embedded on Another Site
If you have a third party site that you would like to embed the survey on (something like WordPress, SquareSpace, etc.), you can click on Integrate in the top right corner of the survey builder, next to the save button.
You will be given an embed code that you can copy to your clipboard by clicking the clipboard icon at the bottom right of the code. You can then paste the embed code into a custom code or HTML element in the other platform.
As a Link Directly to the Survey
When you click on Integrate from the survey builder, you will be shown an embed code or a direct link to the survey. We do not recommend using the direct link to the survey as it is not branded towards your business and is a generic link.
Viewing Survey Submissions
You can view survey submissions within Sites -> Surveys -> Submissions. All answers will also be written to the contact record under the appropriate custom field. More information is in the article Where Do Form Responses Show Up?
Using Surveys with Workflows
You can use survey submissions to trigger workflows and even use survey responses to customize your lead's journey through the workflow.
- Start by going to Automation -> Workflows.
Create a new workflow from scratch by clicking the Create Workflow button.
Click on Add New Trigger.
Select Survey Submitted as the trigger.
Add a filter to specify which survey triggers the workflow. If you don't add this filter, every survey in your account will trigger this workflow.
You can also utilize if-else statements for conditional actions based on survey responses. So for instance, if someone answered yes to a question, you could send them down a different path than someone who answered no to a question.
Add an if/else action to your workflow by clicking the plus sign in the workflow where you want to add the action and then selecting the If / Else option.
Create your appropriate branches using custom fields, so your first branch might be "Would you recommend us" is yes and your second branch might be "Would you recommend us" is no. This would allow you to tailor your workflow to their response to the question and allow you to add different actions to the different branches.
